Project – AI-Powered National T2D Risk Prediction and Personalised Care Pathways
This project will integrate genomic, biomarker, clinical, and lifestyle data from diverse Australian populations into an AI-powered predictive model for Type 2 Diabetes (T2D) risk. The platform will enable early identification of high-risk individuals and provide personalised care plans embedded directly into GP practice software and hospital electronic records. Targeted interventions will be delivered via primary care, telehealth, and community programs, ensuring accessibility across urban, rural, and Indigenous communities. Partnerships with ACCHOs, state health departments, Diabetes Australia, and industry tech leaders will support national-scale deployment. The project aims to reduce diabetes-related complications by 20% within five years, improve equity in prevention, and establish an open AI-health data infrastructure for ongoing chronic disease research.
Call for Partnerships:
We welcome partnerships with universities, health services, primary care networks, technology providers, government agencies, and community health organisations to provide data access, clinical expertise, digital health capacity, and co-funding for large-scale trials.
